Yes it was Marv Levy. Can't remember the year, but he used it because he felt it was the formation that best utilized the talent available. He discusses it in his biography.

I used this book quite a bit as I was writing my Pop Warner book. Very helpful with descriptions of long lost offensive formations.

And that is one I was unaware of ... the answer I was looking for was George Wilson Jr., who wore 48 for Miami in 1966.
So we have three QBs wearing numbers in the 40s after 1960. Am I missing any more?
